Welcome to the Home Page of Pulmonary & Allergy Associates, one of New Jersey's oldest and largest medical practice dedicated to caring for people with lung, allergy and sleep problems. Our highly trained doctors, physician assistants, nurse practitioners, respiratory therapists, nurses, medical assistants and office associates are dedicated to serving your needs. Our "team" approach will provide you with the highest standard of health care.

In order to expedite your registration process and decrease waiting time, please complete the attached patient information forms and bring them with you on the day of your appointment. Please arrive 15 minutes early in order to process your information. In addition, please have the following items available at that time:

Your insurance card(s)
Your insurance referral- If your insurance requires you to have a paper/electronic referral to be seen by a specialist, that referral must be brought with you to our office in order to be seen by one of our doctors. We cannot accept faxed copies.
Any breathing tests, chest X-RAYS/CT scans or test results relevant to this visit. Please bring the reports as well as the actual films.
